Who’s Eligible
Medicare Insured Individuals
- You’re under the care of a doctor, and you’re getting services under a plan of care established and reviewed regularly by a doctor.
- A doctor certifies that you need intermittent skilled nursing care (other than drawing blood) - "intermittent" qualifies as fewer than 7 days/week for less than 8 hours each day for up to 21 days
- You’re homebound, and a doctor certifies that you’re homebound
Medical Insured Individuals
- During the recovery phase of post hospital or institutional discharge or during the recovery phase following an acute episode or exacerbation of an illness of a homebound recipient
- You’re homebound, and a doctor certifies that you’re homebound
Veterans
Enrolled in VA health care, eligible for community care, and meet the clinical criteria for the service
Privately Insured Individuals
Private insurance companies have their own criteria that needs to be met to be eligible for home health services. Please call us and/or your provider to confirm if you are eligible for services.
